Dear Sir,

A baby boy was born to my wife on 30-Sep-1978 under the care of Dr. (Mrs.) Sudha Gupta. A report was submitted by her in prescribed form vide no. 351 silsila No. 29 to Bareilly Municipality. (copy enclosed)

The name of child was mentioned as ‘Baby Sudha’. Now the name of child is ‘Samarth Agarwal’. I had submitted an application for issuance of birth certificate of my son along with desired papers. There is no response to my application since last two months.

Please provide the following information under Right to Information Act 2005.

1. Please advise what is the procedure to get Duplicate Birth Certificate for a person born in 1978?

2. Please send copies/ submit list of documents to be submitted, for issuance of Birth Certificate to my son, if any.

3. Do the officers of Nagar Nigam know that The “Registration of Birth and Death Act (1969)”, was enacted all over India to provide a uniform law for the regulation of registration of births and deaths in India? As per information collected from Indian Embassy in New York this certificate issued within 4 working days?

4. I on 20-June-2007, to get information from Nagar Nigam Bareilly filled the ‘Feed back from’ http://www.bareillynagarnigam.in/English/default.htm. There is no response so far.

5. To view the feedback submitted by me I clicked on the link View All The Given Feedbacks. This opens page -
6. http://www.bareillynagarnigam.in/English/default.htm, which displays Server Error in '/' Application.

7. Please give the names and designations of the officials who were supposed to process this feed back and why have they not done so?

8. Please advise the daily progress made on my feed back and letter, so far. i.e. when did this application reach which officer, for how long did it stay with that officer and what did he/ she do during that period?

9. Please advise what action would be taken against these officials for not doing their work and for causing harassment to the public? By when would that action be taken?

10. Please give the names, designations and qualifications of the officials who are responsible for supervision, running and maintenance of web site of Nagar Nigam, Bareilly?

11. How much was payment was made for development of web site of Nagar Nigam, Bareilly?

12. When the web site of Nagar Nigam, Bareilly was last updated?

13. It seems that web site of Nagar Nigam, Bareilly is copied from web site of Aligarh as at places on page http://www.bareillynagarnigam.in/English/default.htm it mentions Aligarh. Please confirm if this is true.

14. Please advise date wise how many feed backs have been received on your web site and what action has been taken by on the feed backs by the officers of Nagar Nigam.

15. Please give names and designations of the officials of Nagar Nigam, Bareilly who can be contact for assistance along with the numbers of fixed line, fax, mobile telephones and email ID’s.
